What You Need to Know:

  2. Immunizations/Vaccines (Completed MMR & Varicella series and Meningitis ACWY are required for compliance.)
  3. Health Insurance Enrollment/Waiver (Gallagher Student Health Portal)

New incoming and transfer students starting fall semester 2026 will need to comply with health form and immunization requirements by July 15, 2026. Students must upload and complete documents on our student health portal at using your Wesleyan email credentials. Make certain you click "submit" frequently to save your data in the health portal.  You have about 60 minutes with each log in before the system will sign you out so clicking "submit" will assure your data is saved.

Student Athletes: Athletic Department requirements are in addition to the Health Center requirements- you must complete both.  If approved by Athletics for a sports physical at the health center, the cost is $75.00.

Required Forms to be Completed in Student Health Portal: 

  • Log into the Medicat Student Portal and navigate to the Forms tab.
    • Consent for Treatment (See below for additional information)
    • Wesleyan Health History 
    • Tuberculosis (TB) Screening Questionnaire
    • Texting Opt-In Consent

Required Immunizations to be Completed in Student Health Portal:

  • Log into the Medicat Student Portal and navigate to the Immunizations tab.
Domestic Students: We can import your vaccine history directly from your State vaccine registry.
  • Click the Blue Icon to request immunization records from your State registry.
  • Enter your legal name at the time the vaccination was received.
  • If you received multiple vaccines in different states, be sure to enter all residence history before submitting.
  • More information about State Vaccine registries can be found here.
  • Note: This service is unavailable in AK, AR, HI, NH, ME, PA, VT AND DC.
International Students and Students Where Registry Import Is Not Available: You must manually enter your vaccine dates and upload an official copy of your immunization record. 
  • Obtain a copy of your immunization record from your provider's office, which is acceptable for immunization documentation. Records must be translated to English.  If an immunization record is not available to you, this downloadable immunization form can be accepted in lieu of your official immunization record, but must be completed and signed by your Health Care Provider. The State of Connecticut requirements and the University’s immunization policy can be found here.  
  • Do not start entering dates until you have all of your immunization information available to you.  Enter the dates of your MMR, Varicella and Meningitis ACWY vaccine all at once and then immediately upload a copy of your immunization record or completed immunization form.  
    • Note: If you do not upload your record, the dates you previously entered will not save.
  • If a required vaccine is not available for you to receive (ie. Meningitis ACWY vaccine in China), you must send us a secure message in the student health portal to Julia Gardner, Nurse Manager.  We will work with you to coordinate attendance to the vaccine clinic on campus during orientation.
Immunization Exemptions (Updated 12/07/2023)
  • Medical Exemptions: Medical exemptions from vaccines may be requested by submitting the Medical Immunization Exemption Form to the Health Center via the .  The form must be completed by a healthcare provider and will be reviewed by            the Davison Health Center Medical Director.
  • Religious Exemptions: In accordance with , 九色视频 does not offer religious exemptions from required vaccinations.  The University will review stamped and notarized documentation of religious exemptions issued on or before April 27, 2021. Religious exemptions from outside the State of Connecticut or from grades K-12 issued after April 27, 2021 will not be accepted.

Consent for Treatment Form (Required)

  • If over 18 years of age, log into Medicat and sign Consent for Treatment form.
  • If under 18 years of age, download/print form here for both student and parent/guardian to complete. Upload a copy of the completed form in the Student Health Portal.

Health Insurance (Required)

  • Please be sure to review the University Health Insurance Plan and if you have comparable private insurance to waive out at . You cannot complete this step until you receive your Wes email credentials in early June.  Please note: ALL students must waive out or enroll in the University sponsored plan by TBD.  The website will open for compliance on TBD.

  • If you have private insurance coverage, upload an image of your insurance card to the Student Health Services Medicat Portal under "Document Upload".

  • Parents are welcome to visit the Davison Health Center or contact us by phone, but students over 18 years of age are entitled to full medical information privacy protection. The Health Center can discuss details of evaluation or treatment only with the student’s signed consent.
